Ten years ago today, The Institute of Internal Auditors marked its 75th anniversary by ringing the opening bell at the New York Stock Exchange (NYSE) alongside leaders and members of our New York Chapter. It was a proud moment for our profession. During the ceremony, NYSE officials recognized the important role internal audit plays in strengthening governance, transparency, and trust. A decade later, that recognition continues to grow. Every day, The IIA advocates for the profession by working with policymakers, regulators, standard setters, stock exchanges, and other stakeholders to advance the value of internal audit around the world. 🚨 “Internal Audit Didn’t Miss the Collapse.” “It Was Trained Not to See It.” → The reports were completed. → The audits were performed. → The ratings were “Satisfactory.” → The Board received assurance. → Management praised the control environment. → The audit plan was fully executed. And yet… the institution was already collapsing internally. Because the most dangerous audit failure is not incompetence. It is institutional domestication. 📌 Sometimes Internal Audit does not fail because it lacks intelligence. It fails because it slowly becomes psychologically dependent on the very system it was designed to challenge. And once that happens… audit stops auditing reality. It starts auditing comfort. 🧠 The Great Illusion Boards Still Believe Most Boards assume Internal Audit exists to detect problems. That is only partially true. In fragile organizations… Internal Audit often evolves into something far more dangerous: ⚠️ “Controlled Assurance.” A condition where: - findings remain technically correct - reports remain professionally written - methodologies remain compliant …but the function itself quietly loses the courage to confront institutional truth. The audit universe still exists. The independence no longer does. 📌 “Some audit functions remain operational long after they stop being dangerous.” 🎭 The First Sign of Audit Decay: Findings Become Smaller While Risks Become Bigger Watch carefully. This symptom appears constantly before institutional failure. As organizational pressure increases: - audit observations become softer - language becomes diplomatic - escalation thresholds rise - root causes become diluted - severe issues get reframed operationally Suddenly: - governance failures become “process gaps” - fear cultures become “communication challenges” - executive interference becomes “leadership style” - strategic recklessness becomes “risk appetite variation” 📌 “The organization is burning… but the audit report discusses smoke ventilation procedures.” And everyone pretends this is professionalism. ⚠️ The Real Crisis: Internal Audit Slowly Learns Which Truths Are Unsafe Nobody says it explicitly. That is what makes it dangerous. Over time, audit teams quietly discover: - which executives cannot be challenged - which findings create political tension - which topics trigger defensiveness - which reports get careers stalled - which truths the institution rewards you for ignoring And eventually… survival intelligence replaces audit skepticism. This is how institutional blindness gets manufactured. Not through corruption. Through adaptation. 📌 “The most fragile audit functions are not always compromised financially. Cancer fatigue is often not about the cancer. Today, it's this: My appointment scheduled months ago for next week was suddenly moved without my consent. Without any inquiry into whether that worked. And I don't mean moved by a matter of minutes. Different day. A totally different daypart. From 10am Thurs to 1pm Fri. Having spent my entire career in client services, I've never understood how patients are treated. Not the medical treatment. I'm fortunate mine is excellent. But respect needs to be part of healthcare too. Respect that our days still include life outside of our medical appointments, and as we navigate lives already substantially disrupted, calendar control is important. I fully understand that emergencies arise. But this isn't that. This is simply about the precedent that in this situation, there is a power dynamic. One in the patient is expected to be flexible, accommodating, and grateful. All of which I try to be, when I can. But this time I can't. So today I said "no." A word I feel privileged to know I'm allowed to say. A word I share here so everyone knows they have that right too. I'll now be seeing my nurse practitioner instead. I'm also trying to troubleshoot this so no one else is blindsided by a randomly moved appointment. But I'm also exhausted by this back and forth. And most other chronic patients are too. So when you wonder whether chronic illness is exhausting, and why, this is a great example. Invisible. Incomprehensible. Avoidable. And yet more common than we should have to accept. If this resonates, a reminder: you’re allowed to say "no," too.
