That came on top of a difficult childhood marked by neglect, and the kind of isolation that leaves a mark. He grew up on a Minnesota dairy farm, woke up at 4:30 to milk cows, and ate alone through all four years of high school because he looked different from the people around him. A lot of people would have accepted that verdict. Jayme did not. He enlisted in the Army to build a way out and a way forward. That decision changed the rest of his life. He became a combat medic, earned his degree, moved into hospital administration, and eventually led major Army healthcare systems in Europe and in the National Capital Region. Some defining moments are not dramatic on the surface. They are a young person deciding that other people do not get to write the ending. The full conversation is in the comments. Other nights, it takes a little more thought… we dig deeper together to find the bright spots. But we always find them. No matter how easy, hard, boring, or exciting a day is — there are always things to be thankful for. Maybe it’s a big work win, a killer workout, or simply having hot water and electricity (because anyone who’s gone without during a Texas storm knows those aren’t guaranteed comforts!). Really insightful analysis. In theory, “Value Based Care” is incredible. In practice, its turned into: 1. The payers’ way of locking in better risk-adjusted returns by punting the actuarial risk to providers and turning them into mini-insurance companies. This is especially important post-ACA where their profit is capped by MLR restrictions 2. Providers rewarded for investing in risk-adjustment infrastructure & process/input metric maximizations rather than true care delivery innovation Fee For Service and the CPT code set has its limitations, but it’s evolved dramatically over the last 10-15 years and has added a variety of VBC-inspired codes (think CCM, APCM, TCM, RPM). In this “Enlightened FFS” world, you can actually build a sustainable business model because you have revenue predictability, and that may lead to more true care delivery innovation than total capitation ever could.

Radiation Business Solutions
Radiation Business Solutions continues its proactive work addressing payer inconsistencies related to CPT 77387. Following outreach by our team, we connected directly with a Senior Provider Consultant at Capital Blue Cross Senior Blue regarding recent denials of CPT 77387, both with and without modifier 26. During that conversation, Capital Blue Cross confirmed the denials were tied to a delay in receiving updated Medicare guidance, which caused their system to incorrectly flag the code as invalid. After reviewing the current Medicare policy, Capital Blue Cross confirmed that 77387 with modifier 26 is the correct billing format. They have now begun reprocessing all previously submitted claims billed with modifier 26. In fact, one claim was reprocessed and approved for payment during the call. Claims submitted without modifier 26 will need to be resubmitted to ensure appropriate reimbursement. We are incredibly proud of our team for staying on top of evolving guidance, engaging directly with payers, and advocating for accurate claim resolution on behalf of our clients.
