Just wrapped up the second recording today. This one is special for a couple reasons. The first is because it's with my good friend Dr. Ed Zuckerberg and the other is because Dr. Zuckerberg is the first guest to appear on the pod *twice*. He and I spoke at the end of November last year and he was gracious enough to put up with me again. Since he stepped away from clinical practice he's become deeply immersed into the topic of the "Oral Systemic" connection. I don't think it's an exaggeration to say he's one of the world's most knowledgeable doctors on the subject. Our first episode was great and we continued that in this episode with a look at p. gingivalis, salivary diagnostics, and some of the leading edge companies he's advising that aren't yet on the market. Ed gets a chance to see a lot of "behind the scenes" products, especially those in the periodontal category and you'll want to hear about them. On January 1, 2026, they added MIPS Improvement Activity IA_PM_28: "𝗜𝗻𝘁𝗲𝗴𝗿𝗮𝘁𝗶𝗻𝗴 𝗢𝗿𝗮𝗹 𝗛𝗲𝗮𝗹𝘁𝗵 𝗶𝗻 𝗣𝗿𝗶𝗺𝗮𝗿𝘆 𝗖𝗮𝗿𝗲." Primary care physicians can now earn Medicare quality credit for: → Oral health screening on intake forms → Intraoral screenings during primary care visits → Building dental referral networks → Referring patients without a dental home → Documenting oral health findings in the EHR Read that list again. Screenings. Risk assessment. Referral networks. Documentation. That is the same playbook CMS uses for 𝙘𝙤𝙡𝙤𝙣𝙤𝙨𝙘𝙤𝙥𝙞𝙚𝙨, 𝙢𝙖𝙢𝙢𝙤𝙜𝙧𝙖𝙢𝙨, 𝙖𝙣𝙙 𝙙𝙚𝙥𝙧𝙚𝙨𝙨𝙞𝙤𝙣 𝙨𝙘𝙧𝙚𝙚𝙣𝙞𝙣𝙜𝙨. They put oral health in MIPS alongside diabetes management and tobacco cessation, and told primary care physicians: 𝘁𝗵𝗶𝘀 𝗶𝘀 𝘆𝗼𝘂𝗿 𝗹𝗮𝗻𝗲 𝗻𝗼𝘄. Commercial payers have historically followed CMS on quality measurement. When CMS defined value-based care, commercial plans followed. When CMS expanded telehealth, private insurers followed within months. If CMS considers oral health a measurable component of primary care quality, it becomes very difficult for the rest of the system to argue it isn't 🤔 To me, this is a welcome catalyst which paves the way for what's to follow. Researchers gain critical insights into safety, feasibility, patient experience, and clinical performance that can only be understood through human evaluation.
