What if we’re asking the wrong questions during substance use assessments? Too often, intake focuses on the substance—what was used, how much, and when. But far less attention is given to the experiences that often come before substance use begins. Research continues to show a strong connection between early trauma and later substance use disorders. The landmark Adverse Childhood Experiences (ACEs) study conducted by the Centers for Disease Control and Prevention and Kaiser Permanente found a clear correlation between childhood trauma and increased risk for substance use and other long-term health problems (Felitti et al., 1998). Understanding the neurobiology of trauma and how early adversity affects brain development is essential for those working in addiction treatment. Yet many substance use counselors receive limited training in trauma-informed care. The Substance Abuse and Mental Health Services Administration emphasizes the importance of trauma-informed care in behavioral health settings. Recognizing trauma, creating safety, and addressing underlying experiences can significantly improve treatment engagement and long-term recovery outcomes. As professionals in the field, continuing to expand training and awareness around trauma-informed approaches can help us provide more effective and compassionate care.
