After helping resolve her issues she was very happy.

    Objectives: This is a prospective study to evaluate the optimal criteria for diagnosing primary breast carcinoma with pre-operative FDG PET-CT scan

    Methods: 20 patients are randomly selected from a group of patient who had pre-operative FDG PET-CT scan performed from 9/2003 to 4/2004 followed by segmentectomy or mastectomy for primary breast carcinoma. The indication for pre-operative PET-CT scan performed are (i) palpable axillary node (N1),or (ii) large primary tumor ≥ 5 cm (T3), or…

    Objectives: This is a prospective study to evaluate the optimal criteria for diagnosing primary breast carcinoma with pre-operative FDG PET-CT scan

    Methods: 20 patients are randomly selected from a group of patient who had pre-operative FDG PET-CT scan performed from 9/2003 to 4/2004 followed by segmentectomy or mastectomy for primary breast carcinoma. The indication for pre-operative PET-CT scan performed are (i) palpable axillary node (N1),or (ii) large primary tumor ≥ 5 cm (T3), or (3) inner quadrant primary tumor >3 cm. Body PET-CT scan is performed at 45 minutes (early scan) and at 2 hours (delayed scan) after injection of 12 mCi. FDG /1.73 sq.m. and serum glucose corrected, using a LSO Biograph PET-CT scanner. SUVmax of the breast lesion, the liver, ipsilateral nipple, contralateral nipple was obtained. The study will test the sensitivity of the various criteria in diagnosing the primary carcinoma.

    Conclusions: The study shows that for primary breast carcinoma, the usual criteria of lesion SUVmax ≥ 2.5 commonly used in other tumors has a low sensitivity of 70%. The best criteria to improve sensitivity is comparing the lesion FDG metabolism with the contralateral nipple metabolism and using the primary lesion / contralateral nipple SUVmax ratio of ≥1.25, then the sensitivity of diagnosing the primary carcinoma in this selected group of patient is 100%. The use of the contralateral nipple will avoid the hypermetabolism sometimes present in the ipsilateral nipple near the primary tumor.
