📑 FDA’s new draft guidance on oncology endpoints says it out loud: tumor shrinkage isn’t the win. Survival and real-world outcomes are. ☢️ Radiopharma could deliver exactly that—but let’s be honest: it’s stuck in a system not built for it. Supply: 3 curies of Ac-225 a year is a rounding error in oncology. GMP: hot cells and shielding don’t scale like stainless steel tanks. Logistics: a 10-hour half-life (Pb-212) makes “just-in-time” a literal survival issue. Reimbursement: we’re pricing treatments like they’re line items, not system-wide value creators. ⚡ Radiopharma won’t scale until we stop treating it like a biologic with isotopes bolted on—and start rethinking the entire value chain. 📌 The FDA has raised the bar on what endpoints matter.
