At CVS Caremark, advancements in automation, including touchless prior authorization, and interoperability are helping accelerate decisions and reduce administrative burden for providers. In some cases, when clinical criteria are met, specialty medication requests can now be approved in as little as 22 seconds, demonstrating how a more efficient, responsive system is taking shape. We're continuing to simplify the experience and improve approval speeds to create less burden for providers, fewer delays and faster access to care for patients. And get THIS. 🚨 Ken developed the Silverstein Fire Risk Assessment Tool, which has transformed surgical safety protocols, reducing operating room fires and improving team communication. His innovation has made a huge impact, yet he remains incredibly humble. (I mean, Google it. It’s a real thing!) In addition to his work in surgical safety, Ken has held leadership roles like Vice President of Clinical Operations at Walmart Health and Chief Physician Executive at ChristianaCare. Ken Silverstein, MD, MBA liked thisKen Silverstein, MD, MBA liked thisAs a kid growing up, my favorite TV show, without question, was Emergency!. It’s co-star, Randolp Mantooth died yesterday. Playing the role of Johnny Gage, he captivated my imagination as a child, a modern day hero - brave, creative, and there to help others in their worst moments. When the show premiered, there were 12 paramedic units in all of North America. In the next three years, 46 states enacted laws that allowed paramedics to practice emergency medicine. Within 10 years, more than half of all Americans were within 10 minutes of a paramedic rescue or ambulance unit. My first job in healthcare was as an EMT. For six years through college and the summers that followed, I lived out my version of Johnny Gage. Perhaps not as daring or courageous, but I’d like to think that I helped many people on their very worst day. It kindled in me a desire to go to medical school, and well, the rest is history. When Randolph Mantooth was offered the job, it’s said he had no idea what a paramedic was. He didn’t want the job. I’m glad he took it. For the tens of millions of patients helped each year by EMS, and for this very appreciative doctor, thank you, and may you rest in peace.

As I close my chapter as interim Chief Medical Officer at Heritage Health, an FQHC serving more than 25,000 patients and families across northern Idaho, the work I'm most proud of happened at the intersection of leadership and operations. Two things, specifically: Building a clinical leadership structure — clear accountability, defined roles, and a framework for how clinical decisions get made and communicated across the organization. And functionally integrating the clinical and operational leadership teams — creating the conditions where CMO and COO aren't parallel tracks but a genuine dyad, working from the same information toward the same goals. These aren't glamorous deliverables. There's no ribbon to cut. But the downstream effects are real: better communication, clearer prioritization, sounder decision-making, and most importantly, providers who feel heard and supported in their work. That's the foundation everything else builds on. If your organization is navigating a CMO transition, or simply recognizes that tighter clinical-operational alignment could strengthen what you’re building, I'd welcome a conversation.

The committee will advise the federal government on practical policies that aim to reduce friction within the system, improve experiences for Medicare and Medicaid beneficiaries, and rid the system of fraud, waste, and abuse. It’s a broad agenda, and a unique opportunity for me to share insights from a career that has traversed many sectors within the healthcare ecosystem. Truly, I didn’t. I just knew I cared deeply about people, leadership, and helping others reconnect to who they are when pressure, stress, and expectations start pulling them away from themselves. Over the years, I’ve had the privilege of sitting with executives, leadership teams, founders, and individuals during some really challenging moments. "How does the current financial structure of healthcare create disincentives for quality patient care?". This question assumes that we generally believe that the quality of patient care is disconnected from the price we pay. A key question to ask would be how many intermediaries are involved in a healthcare economic transaction. First party payment: When you purchase an item for you with your money, it is first party payment. This transaction tend to be most sensitive to the quality of the item. (Example: me buying a pair of sunglasses for myself or me paying a monthly membership to my DPC doctor for routine care) 2nd party payment: When you purchase an item with your money for some one else, it is second party payment. Quality of the item may or may not be the most critical consideration. (Example, father buying a game app for your teenager reluctantly) Third party payment: When you purchase an item with someone else’s money for some one else, it is Third party payment. Quality of the item is unlikely to be on the top of the checklist for this transaction. (Example, agents who sell insurance to employers who don’t really know how to negotiate quality and value for their employees health needs. Disclaimer: there are many TPA's on this forum - among my LinkedIn friends who puts quality very high on their priority but they are not your traditional TPA's). 4th party payment? System where no quality item is delivered but someone’s money is used to buy someone else’s item to sell to some one else but all parties except the economic buyer becomes richer. This is the 4th party payment type. Example: traditional American healthcare. Quality of care is inversely proportional to the number of layers in the transaction. This is why 1st party transactions employed by direct care practices such as DPC always wins when it comes to patient satisfaction. (But the 4th party transactions are significantly more profitable for the 4th party. Case in point: BUCA. A single BUCA takes home $6-10B per quarter in earned income while care quality for patients gets progressively worse and unaffordable). Would you buy your next grocery list via 4th party payment or first party payment? Why? In the name of scale, our policy makers have traditionally trusted the 4th party payment system for our care. This mandates us to pay whatever the 4th party asks us to but the 4th party has no obligation to deliver because we the patients are not their direct customer- Policy makers are.
