The Muldoon family, covered by IBX through Steamfitters Local 420, shared their son’s story of breakthrough care at Children's Hospital of Philadelphia — made possible in part through their health plan. KJ became the first patient to receive a custom, life-saving gene therapy for a rare condition. Their story is a powerful example of the kind of access we strive to deliver every day. Then, Joe Geist welcomed IBX President & CEO Kelly Munson to the stage to talk about where IBX is headed and how we’re getting there — emphasizing our role in convening stakeholders across the health care ecosystem, working differently with providers to drive solutions that improve affordability and access for our communities. Sally Kim presented research from Advisory Board on today’s biggest cost drivers and what strategies health plans and employers are using to maintain affordability without compromising access or outcomes. With help from Joe and Alexandra Jorgensen, our Sales team led engaging peer roundtable discussions with leaders from different organizations on key benefits-related topics. Reetika Kumar MD, FACP and Joelle Parks from Optum Rx, discussed our new future-ready PBM model and how it’s designed to bring greater flexibility and cost control to pharmacy benefits. Joel Enterline liked thisJoel Enterline liked thisRight now, some captive results may look stable for the exact same reason the broader stop-loss market looked stable before the hard market arrived. The deterioration had not fully surfaced yet. Captives can create the appearance of stability while risk is quietly being redistributed underneath the structure. Retained layers. Corridors. Aggregates. Attachment point movement. Collateral structures. Quota share participation. Reinsurance layering. The volatility does not always disappear. Sometimes it simply moves. That matters because financial stability and risk stability are not always the same thing. A captive can report relatively smooth performance while severity is still developing underneath the system, especially when risk is spread across employers, captive layers, stop-loss carriers, and reinsurers over multiple years. Experienced operators know where to look. Attachment migration. Increasing corridor utilization. Growing collateral pressure. Shifting laser activity. Rising pooled claim frequency. Members staying high cost longer. Renewal increases that appear disconnected from reported financial stability. Those signals often emerge before the financial statements fully reflect the deterioration. And in a market where severity is evolving faster than many pricing models were designed for, the ability to reposition risk inside a structure can temporarily delay how quickly underlying pressure becomes visible. That does not mean captives are bad structures. Some are exceptionally disciplined. But it does mean outsiders often underestimate how much expertise is required to determine whether risk has actually improved, or whether it has simply been redistributed across layers where it is harder to immediately see. The market spends a lot of time analyzing reported outcomes. Less time analyzing where the risk actually went. Industry pro's : thoughts on this?

I joined Julia Boorstin on CNBC to talk about it, one week after my wife Katy Shah and I welcomed our second child, Miki, into the world. Personally and professionally, a week I won't forget. We started Thyme Care six years ago, and today we have 120,000+ individuals with cancer in our care. 8 million Americans are able to reach us. I'm proud of the scale, because scale means more people getting real support during the hardest moment of their lives. But the best part of it all is the team I get to do it with and everyone who chooses to make Thyme Care their team. I've worked in oncology for 17 years. I have never been part of a team like this, and I'm energized by what we'll build together in the years to come. https://lnkd.in/e9ZVCvDy
