Almost always, that conclusion traces back to one number: investment per provider. That number is incomplete. And in a world where integration is the strategy, acting on it has real consequences. AMGA Consulting's new white paper makes the case that system-affiliated medical groups are consistently judged by a metric that doesn't reflect how physician integration actually works financially. Ancillary revenues shift to the health system. Place-of-service changes reduce professional fee revenue while increasing total organizational revenue. These aren't signs of underperformance. They're artifacts of integration itself. When you normalize for those differences, the bottom-line performance of system-affiliated and independent physicians is nearly identical. That reframing matters, not just financially, but culturally. When medical groups are seen as cost centers, physician engagement suffers and the strategic value of the physician enterprise gets overlooked at exactly the moment it is becoming more important. The health systems that thrive in the next decade will be the ones that see their physicians clearly, as a strategic asset, not a cost to be managed. Download the report here:

Jill Powelson, DrPH, RN shared thisRecently, I presented on the topic of trust within organizations and posed a poll question to the audience: "How would you rate the level of trust between providers and administrators in your organization?" Thankfully, none of the respondents selected Very Low, but a total of 78% rated their levels of trust as Low or neutral (Neither Low nor High). Only 22% indicated High or Very High levels of trust. A few themes resonated throughout the afternoon: ☑️ wRVU and reimbursement changes relative to the 2026 CMS Physician Fee Schedule are live, and many organizations haven't adjusted their comp plans to reflect them. ☑️ Anesthesiology is one of the most challenging specialties for provider compensation management, particularly surrounding supervision and call play dynamics. ☑️ APPs account for a significant portion of the provider workforce, yet strategic, contemporary compensation frameworks lag behind. ☑️ Organizations that truly engage physicians as partners build something genuinely high-performing — and the difference is visible in culture, retention, and patient care. Jill Powelson, DrPH, RN shared thisAre your work relationships true or transactional? With work-from-home, the gig economy, and - for many of us - shorter stints with employers than in our parents' generation, it's harder to engage with co-workers and easier to fall into a pattern of work relationships that are more about "what's in it for me?" or "what have you done for me lately?" Organizations can slip into emphasizing profits over people. In summary, too often, our work relationships have become more transactional than true. Healthcare organizations can also suffer from this shortcoming, at times viewing their employed providers in a transactional way. Building real instead of transactional relationships at work - including with providers - is one of the keys to a high-performing healthcare organization. The great Edgar Schein, culture guru extraordinaire, taught that the term “culture” is amorphous and challenging to measure. If we try to explain culture, many people will not know what we are talking about. Key point: Instead of attempting to define and discuss your culture, focus on behaviors and norms. Are they what you want within your organization? If not, consider how to move them in a more positive direction rather than directly trying to tackle culture. I have the privilege of working with Jack Silversin and Mary Jane Kornacki, authors* and consultants, who have been successfully advising healthcare organizations for 30+ years. Jill Powelson, DrPH, RN shared this“I Don’t Hate You. I Hate Your Role.” I had started a new position as a service line administrator, and I was full of enthusiasm. One of the physicians in the service line asked to meet with me. He was clearly not happy. In the course of our discussion, he confided that while he did not hate me personally, he hated administrators in general. His statement caught me off guard – hate is such a strong word. I have always prided myself on being helpful to and supportive of the providers I worked with. But he did not trust or like administrators, and after thinking about it for a long while afterwards, I could understand why. Before I arrived, he was told “no” too often … no, you can’t have a raise … no, we can’t hire more help for you … no, we can’t pay for you to attend that conference. Decisions were not explained well to him, so he didn’t understand “the why” of what was happening when it affected him and his practice. He didn’t feel that his voice was heard. He felt like he was doing all of the work, and we – the administrators – were making all of the decisions. Bad decisions at that. He did not trust administrators, and he eventually felt hate toward the role. A couple of years later, he retired early. That was a shame, because he was a skilled and caring physician. That was a tough conversation that I still reflect on more than 20 years later. If I had the experience and knowledge then that I do now, I believe we could have had a different outcome with that physician. Had we had a strong Provider Compact in place and been “living it” on a daily basis, our physicians and administrators would have been better positioned to work together. I am thrilled to enter the next phase of my career in the MUSC Department of Healthcare Leadership & Management (DHLM). We are on the cutting edge of innovation in healthcare management education and poised to prepare the next generation of healthcare leaders for dynamic roles in a fast-changing environment. I have spent four wonderful years as the Program Director of the MHA and MSHA Programs at Virginia Commonwealth University. The incredible colleagues, students, alumni, and community partners I worked with will always hold a special place in my heart. I have learned so much from everyone and become a better person, educator, and administrator from this experience. To the students I have taught and recruited at VCU: Always remember that you earned your place and worked hard to achieve your goals. I am so proud of the professionals you are becoming. From orientation through residency, I have followed your steps with anticipation and pride. I know you will do great things in your lives and hope that you will always strive to do your best and help the communities that you serve. I am honored to have shared the classroom with you and, hopefully, made Health Economics and Health Policy interesting, if not fun. I will always be a resource for you. We just released the AMGA 2026 Medical Group Compensation and Productivity Survey, and one finding keeps coming up in every conversation we’re having with group leaders: Nearly half of compensation growth is being funded by higher provider productivity, not gains in reimbursement. wRVUs rose 2.4% in 2025. I’m grateful to my family, friends, colleagues, and coaches/mentors who have supported and challenged me to get to this point. Looking forward to enjoying the day and being intentional about charting the next course or, just maybe, seeing which way the wind blows!

After months of research, Nicole Shea, MHA and I published a white paper that goes beyond the headlines. Here's what we kept coming back to: 🔹 Self-reported survey data and organization-reported data tell very different stories. The gap matters — a lot — when you're designing or benchmarking a comp plan. 🔹 Private equity now controls roughly 1 in 3 anesthesia practices nationally. The financial upside is real. So is the fallout: turnover, burnout, and rising costs that eventually land on health systems. 🔹 Call structure is quietly one of the biggest drivers of dissatisfaction — and it's not always addressed clearly in contracts. 🔹 The care model (medical direction vs. supervision vs. CRNA-only) has a massive impact on productivity and comp — and most benchmarking tools don't account for it. We pulled from AMGA's proprietary national dataset — one of the largest organization-reported benchmarking sources in the country — to ground all of it in real numbers. Kevin brings a wealth of executive leadership experience from major health systems across the country. Most recently, he served as CEO of Florida Medical Center in Fort Lauderdale and previously as CEO of Vista Health System in Waukegan, Illinois. His impressive career also includes serving as President & CEO of Erlanger Health System in Chattanooga, a seven-hospital, $1.2 billion organization and as CEO of Methodist University Hospital in Memphis, where he also held a faculty appointment as Assistant Professor in the College of Medicine at the University of Tennessee.
