Felix Brann shared thisReflecting on an incredible few days at the CMS Quality Conference. In industry we tend to groupthink that the government always moves slowly. What I saw was the polar opposite - CMS leadership is engaging directly with how AI and modern data infrastructure can reshape quality measurement, and doing so in a practical and grounded way. Thanks to Dora Hughes, Michelle Schreiber and teams for convening such a wonderful group. A few takeaways that stuck with me: • CMS clearly sees the potential for AI to close long-standing data gaps in quality measurement - especially by incorporating unstructured data without increasing manual burden. • There's a broad shift toward measuring what actually matters - with a smaller number of measures, grounded in clinically meaningful data rather than claims data. • Quality data shouldn't be a reporting silo - it should be real-time and valuable at the point of care. I feel hugely grateful that we get to work on technology to keep patients safe in this moment. Excited for the future under this group's leadership. Oh and huge thanks to Mindy Riley for all the coordination!

Felix Brann shared this2025 was a year of incredible progress at Pharos (YC S24)… and an exhausting amount of legwork. Building in healthcare is hard. Health systems are rightly cautious about new technology and change when people’s lives are on the line. Legal and security reviews are long and complex. Implementations demand deep flexibility. And AI governance has recently added a new step to the journey. If you’re building a healthcare business expecting a SaaS-style, low-touch sales motion, you’re going to have a bad time. The only way to win is by building durable, trust-based relationships with customers. And the best way to do that is still face to face: fly to meet people where they are, shadow the teams you’re building for, and go to enough conferences that your desk disappears under a pile of badges... It's a lot of effort, but the upside is huge - instead of chasing SaaS deals to outpace churn, you end up building enduring partnerships. Your customers teach you their real pain points, and your product grows to meet them. There is enormous opportunity for AI to create value in today's healthcare system, and the clearest way to see it is standing shoulder to shoulder with the people doing the work. We’re building in health because we can see the future, and we know it’s worth it. Here’s to wearing out the shoes in 2026!

One theme dominated: healthcare desperately needs accurate clinical data, but we're drowning clinicians in documentation burden. Data is useless unless it is accurate, yet the two common "fixes" both fail in the real world. Adding more structured fields increases clinician documentation burden and operational cost. Relying on manual abstraction is expensive, slow, and hard to scale. And oversimplifying measures so they fit structured data undermines the whole purpose of clinical registries, stripping out nuance needed for research, risk adjustment, and subgroup analyses. Pathology makes this tension especially stark because the clinically relevant concepts live in messy narrative reports. Defining an initial cohort from billing codes creates huge false positives. Take CPT 88305, a "single gross and microscopic tissue exam" code spanning everything from colon polyps to skin punches to prostate cores. A patient biopsied years ago for prostate tissue can later be swept into a skin melanoma measure unless a skilled reviewer manually rules them out. Until now, that manual exclusion step was the only reliable fix for orgs like the College of American Pathologists (CAP) Another layer of noise comes from uncertain and highly variable language in pathology reports. Pathologists hedge when morphology is borderline or tissue is limited ("suspicious for," "cannot exclude," "favor"), and those nuances matter for measure eligibility. The same clinical concept is expressed dozens of ways across institutions and individuals. Without robust normalization, keyword and rule-based extraction either miss true cases or amplify false positives. Some measures become so brittle that they are effectively intractable; CAP previously had to abandon automatic collection of an intestinal metaplasia measure because existing approaches could not handle the narrative complexity. At CMSS we presented an AI system with explainable, auditable reasoning that preserves clinical nuance while automating abstraction end to end. Across four CAP measures (about 2,000 reports), the system matched human performance, with low-variance outputs and fully reviewable reasoning for each decision - enabling complex measures that were previously intractable. We are grateful to the CAP, and to the abstractors and managers at Kiser Healthcare Solutions, LLC who built the gold labels. Crazy that a chance meeting last year led us to the main stage this year. So much gratitude for our incredible collaborator Colleen S. who juggled *two jobs* while running this study with us, and for Helen Burstin who has been such a generous champion of innovation in quality and safety. Stay tuned for the paper, but the headline results: Pharos (YC S24) AI had the same accuracy and IRR as two skilled human abstractors, across 2000 independently double-abstracted records. Felix Brann reacted on thisFelix Brann reacted on thisUsing more than 34,000 paired chest CT scans and echocardiograms, we developed and validated an AI model that can identify patients with reduced ejection fraction directly from routine, non-gated, non-contrast chest CT scans—images often acquired for completely unrelated reasons. This work highlights the power of opportunistic screening using data already collected in routine care to detect disease earlier and potentially improve outcomes. It is an exciting example of how AI can bring new clinical value to existing workflows. Proud to have collaborated with an incredible multidisciplinary team. The headline: in a Nature Medicine study out of NYU Langone, two specialized clinical tools — OpenEvidence and UpToDate Expert AI — were benchmarked against three frontier models (GPT-5.2, Gemini 3.1 Pro, Claude Opus 4.6). The general-purpose models won all three evaluations. In one of them, the clinical tools scored no better than Google's AI Overview. OpenEvidence pushed back hard, flagging methodological flaws, including data leakage, and alleging an undisclosed conflict of interest among the authors. Over the weekend, sides were chosen. It's worth getting into the methods. Three benchmarks: 500 MedQA board-style questions and 500 items from OpenAI's HealthBench, both scored by a panel of LLM judges; and a third set - 100 real clinical queries pulled from actual physician use at NYU - graded by 12 blinded clinicians on correctness, completeness, safety, and clarity. But the commentary that stuck with me wasn't about the scores. It's the question of whether answer quality is the primary metric clinicians use to decide whether to adopt a tool. We saw the same debate play out in the early days of ambient scribes: why buy a product when you can hack together a ChatGPT wrapper that does roughly the same thing? The answer turned out to be that clinicians don't just want an accurate output — they want something that is both TRUSTWORHY and they want a SYSTEM that's tuned to their workflow. We had an oversubscribed event with roboticists, engineers, designers, students, kids, grandparents all in one place, seeing for themselves how home robots will change the way we live for the better (video in comments). And they loved Isaac 1's home-first design, which was also recognized with an Industrial Design award by San Francisco Design Week. We cannot wait to introduce him to even more people.

I was listening to this https://lnkd.in/giycVEkG and had a thought I can't shake: What if foundation models are AI's genetic code? DNA gives us instincts... babies cry, calves walk to milk without learning. Foundation models feel similar: pre-loaded with language and reasoning. But evolution doesn't create one perfect organism. It creates fish, mammals, birds... each specialized. Maybe AI works the same way? Not one AGI, but multiple species and individuals that learn and evolve. If foundation models are our "genetic code"... we need to be careful what we embed there. These become instincts for all future AI. Like Sutton says, learning is fundamental and the next crucial piece: how do we take what AI systems learn and feed it back to the next generation? In biology, successful traits get passed down... what's the AI equivalent? Maybe we're not just building better AI... maybe we're designing artificial evolution.

The good part of going public is that you get fresh money and you have a chance to move the market. The bad part is that you have to be much more transparent and open. And that's specially complicated in a nascent industry like #quantumComputing IonQ has seen another report from a shortseller that (in a biased way) sheds some light into the realities of revenues and claims on the public companies on quantum. This, and your usual plethora of news on your #quantumPirates newsletter! Link below: The National Reconstruction Fund Corporation’s $20 million investment in Diraq, which is advancing to stage B of the United States Defence Advanced Research Projects Agency’s Quantum Benchmarking Initiative. QuEra Computing Inc., in partnership with Roadrunner Venture Studios and the State of New Mexico, announced a $4 million initiative to launch a quantum testbed and new facilities in Albuquerque, supporting the state’s $300 million effort to build a globally competitive quantum economy. Meanwhile, the European Union’s selection of the SUPREME consortium, led by the Fraunhofer Institute, targets building up scalable superconducting quantum chip capabilities to enhance Europe’s industrial value chains and quantum competitiveness. On the software front, Qunova released its Hybrid Iterative Variational Quantum Eigensolver algorithm on the AWS Marketplace. IonQ’s acquisition of Seed Innovations highlights ongoing convergence between artificial intelligence and quantum technologies, but at the same time they saw a concerning report from short seller Wolfpack that accuses them of information manipulation. The Ethereum Foundation’s formation of a dedicated Post-Quantum team, led by Thomas Coratger, signals industry-wide recognition that quantum threats to blockchain security are now an engineering challenge; test networks for post-quantum signatures are already operational, with major ecosystem players like Coinbase also advancing defenses. A major research advance was reported with the first comprehensive GPU-accelerated implementation and benchmarking of sample-based quantum diagonalization for quantum chemistry, achieving 95-fold speedup per node and demonstrating strong scaling on state-of-the-art heterogeneous supercomputers such as Frontier.
