Christian Moreno shared thisLilly Direct success is demonstrating that pharmaceutical manufacturers are making substantial gains in the "Direct to Consumer" model. They have now have the ability to fulfill orders at scale (and all the infrastructure and processes behind that endeavor) to consumers' home - with zero involvement from the classic "PBM" intermediary. While this might seem benign and limited to a few medications paid for by consumers directly, when we overlay the scale of some of the recent direct to Self-Insured Employer Plans efforts, one can see potential for real pricing reckoning (see article below for more on investment into the employer direct space). The PBM effectively creates a "formulary" for employers to buy in aggregate - using a pricing strategy that looks at the spend/rebates overall rather than particular drugs. This is why there is often dramatic discrepancies in cost to employer plans vs the "cash" price. When/if the pharmaceutical manufacturers scale up and begin to capture some of that market cost/value upstream, it could be very disruptive to the PBM pricing model. Christian Moreno shared thisI spent the majority of the last week traveling around the country speaking with Kellogg Alumni/business leaders about the concept of "trust" - with Professor Andrew Sykes. The workshops and subsequent conversations were invigorating. In short, I sponsored these sessions in several cities becuase of the clear tie in to the world of "Total Rewards" and health and welfare programs offered by many of our customers here at Lockton. The value of the benefits offering of any employer is often diminished or increased the AMOUNT of trust the employee has in the messenger. How employers articulate the value of the program - how much trust they establish in the process - has an incredibly powerful impact on the employee percieved/experienced value of the program overall. Lockton's communications team is industry leading in designing bespoke, multi modal, "trust building" communications programs. Christian Moreno shared thisMy friend - former colleague - and Kellogg professor - Andrew Sykes gave a powerful keynote in the beautiful Polanco area of Mexico City. Getting to meet so many fellow Kellogg School of Management Alums in Mexico was a reminder of the amazing people that Kellogg puts through their amazing program (present company excluded obviously)! Andrew’s talk on establishing trust is fundamental to so many aspects of my business - and that of my customers in Total Rewards/Strategic Benefits design. For example, the employer - employee trust relationship is either augmented (multiplied) by the quality and communication efficacy of their Health and Welfare benefits; or it is diminished if done poorly or incorrectly. Further, employee benefits broker/consulting role can be often viewed as a commodity - not materially differentiated by the choice of partner. However, as I think about the trust established between Lockton and our clients - the relationship and trust effectively removes/defeats that commoditization. This translates into the sought after “client retention” metric. It is Trust - plain and simple. I look forward to hosting/sponoring Andrew and his talk at Kellogg Alumni events in several cities across the U.S. in the coming months. Christian Moreno posted thisI talk with clients frequently about the benchmarking of their medical plan offering and costs - against either regional or national competitors/like employers. In one of these discussions last week, we flipped the paradigm of the exhibit - and thus the discussion. Instead of plotting the cost of coverage and relative “Actuarial Value” (aka “A.V.”) of the plan from the employee’s perspective, we looked at it from the EMPLOYER’S perspective. How much does it cost the employer - Per Employee Per Year (aka PEPY) - to offer the respective plans; and then compared that to competitors/like employers. The analysis is eye opening. Essentially, it showed the aggregate cost for each employer to offer the same AV plan to their employees. It highlighted the additional “frictional” costs in their benefit plans that is not seen when comparing strictly the employee’s preview. For this exercise, we saw 10% difference in PEPY from one employer to the next. This is huge when priced into the overall cost of Total Rewards/labor costs. The drivers in the difference in PEPY cost could be a number of factors (region, demographics, network selection etc) - but simply understanding that the costs are materially different is informative and can inform strategy.

Today I'm thrilled to share that First Street is being acquired by MSCI Inc. to do that at scale! We started by democratizing climate risk data, putting it on the real estate portals where millions of people interact with it on a daily basis. Then we took that same science to financial markets and built a new category, Climate Risk Financial Modeling (CRFM), faster than I thought possible. The growth has been exponential. Now we will power that impact with the scale and reach of MSCI. That means climate risk moves into the workflows of the world's leading investors, lenders, and insurers, embedded directly into how capital is priced and allocated. When the market reacts to the cost of climate faster, capital moves faster, and the world adapts faster. That is the win, and it is a global one. But none of this happens without people. To the First Street team: you turned a conviction into a category. I am in awe of you. I'll reach the tag limit if I mention all of you so all I will say is: LFG! Dallas Children's Advocacy Center coordinates the investigation, prosecution and healing services for the most severe cases of child abuse in Dallas County - those that rise to the level of a criminal offense. Each year DCAC serves over 6,500 children (and their non-offending family members) who were sexually abused, severely physically abused, or who had witnessed a violent crime. Our average client is a 9-year-old girl, sexually abused by someone she knows and trusts.

DCAC houses the Child Abuse Unit of the Dallas Police Department, six units of CPS, and a Dallas County Assistant District Attorney. Having all of these professionals under one roof drives collaboration and communication in the very sensitive cases that DCAC coordinates. DCAC has 32 partners including 27 law enforcement agencies in Dallas County, Children's Health, the Dallas County District Attorney's Office, the Dallas County Southwestern Institute for Forensic Services, the Texas Department of Family and Protective Services (Child Protective Services, Child Care Licensing, Adult Protective Services), and the University of Texas Southwestern Medical Center at Dallas - Department of Pediatrics. -

As PwC reports this week, healthcare inflation is expected to come in at 9% next year, representing more than double the broad rate of inflation in the US economy. At this rate, the US healthcare market will top $9 trillion by 2035. A significant driver of this continued high rate of inflation is the continued lack of transparency in the cost of both services and drugs in the US healthcare market. And with US provider systems consolidating their market power and US health insurers doing the same, it appears this will only continue as these large incumbents continue to flex their muscles and limit choice. At Collective Health, our average annual healthcare cost inflation—or trend as it’s called in healthcare—is expected to be around 4%, which is where it has hovered over the last few years. And we don’t own a network or a PBM or a hospital or provider groups of any kind.
