Three years ago, Alina A. and I decided to change the narrative by building a community of women investors, VCs, angels, and CVCs. We quickly realized that building a true community is much more than email lists and networking events. To really connect, we need time to talk about life, share interests beyond our investment portfolios, and get comfortable with one another—over good food and cocktails, of course! We focused on hosting small, curated gatherings where our only goal was "friendmaking." As we approach our three-year anniversary, we are proud of the many successes we’ve celebrated: from sharing panel opportunities and co-investing to going on long hikes and swapping recommendations for everything from personal trainers to physicians. We’ve been enriched, both professionally and personally. Thank you all for helping us co-create a community that stays as far from "shark-infested waters" as possible. This time’s theme was Difficult Decisions. In keeping with the theme, Dhana Pawar from Sage and Veda sponsored the difficult task of selecting and naming the best perfume from five outstanding candidates. But for decades, American healthcare has treated it like it does: one office visit at a time, with everything in between left to chance. That's beginning to change. The Centers for Medicare & Medicaid Services has built reimbursement pathways through Remote Patient Monitoring and Advanced Primary Care Management that support a fundamentally different model: continuous care instead of episodic treatment. What's been missing is a platform capable of delivering that model at scale. Cadence is building that platform. By combining a physician-led care model with a Clinical Intelligence layer embedded directly into health systems, Cadence helps clinicians identify risk earlier, automate routine workflows, and focus their time where it matters most. In a single year, partner health systems returned roughly 85,000 clinician hours while resolving 99% of alerts without physician escalation. Just as importantly, Cadence has built an evidence base that's unusual for a company at this stage. Three peer-reviewed studies published in JACC, NEJM Catalyst and Mayo Clinic Proceedings demonstrate meaningful improvements in patient outcomes and reductions in total cost of care, a foundation we believe creates a durable moat. The investment was led by General Partner and Head of Healthcare Robert Mittendorff, MD, MBA, alongside Partner Adam Seabrook and investment team members Nicholas Whitehead, Athena Shea and Keegan McGuire. I started FamilyWell because of what I was seeing in the perinatal space, the way behavioral health was consistently treated as secondary even in a window of care where we know the stakes are high. But the longer we’ve been doing this work, the more I’ve heard from OB/GYNs with patients on the other side of the spectrum - women navigating perimenopause and menopause who are experiencing mood changes, sleep disruption, cognitive shifts, and anxiety. Providers feel like they have such limited options, if any at all, for how to support women in this stage. The Menopause Hub launches today with two resources I’m proud to share: A free clinical white paper I co-authored with Dr. Marissa Caan, MD and Dr. Melissa Sherman, MD, FASAM, FACOG, offering providers a practical evidence-based framework for screening and managing mood disorders during the menopause transition. Open enrollment for the Menopause Behavioral Health Certification (MBH-C), a 15-module interprofessionally accredited program for the coaches, therapists, social workers, and clinicians who are on the front lines of this care every day. Nearly 400 providers have already joined the waitlist. For physicians and nurse practitioners, we also opened the waitlist for our Menopause Behavioral Health Physician Certificate (MBH-PC). Anna Wang liked thisAnna Wang liked thisMore money than ever is going into mental health. Are we getting our money's worth? Over the last decade, employers invested heavily, often by carving mental health out into a real benefit. They expanded access, normalized asking for help, and put support on the table for millions. Then the bill came due. Mental health is now a top-five driver of commercial health spend, growing 4x faster than overall healthcare costs. More therapy, more sessions, more access, and outcomes have barely moved. Here's the uncomfortable part. The model is working exactly as designed. The system routes almost everyone to the same answer: therapy with a clinician, regardless of what they actually need. According to SAMHSA, nearly half of adults receiving mental health treatment don't meet criteria for a diagnosable condition in a given year. Many get more support than their situation calls for, the people in genuine crisis can't get enough, and no one in the loop is paid on whether anyone gets better. That's not a spending problem. It's a care allocation problem. I know, because we spent a decade proving the other way works, in substance use, the hardest corner of behavioral health. We matched care to need, brought the cost of treating someone from roughly $30,000 to less than $3,000, and put 100% of our fees at risk on outcomes and ROI. The structural problems we see in mental health are the same ones we faced early in substance use: a delivery model that rewards billable sessions over outcomes, deep unmet need in high-acuity populations, and incentives with no clear line of sight to ROI. Today we're introducing Sona, a new voice in mental health. It's there in the everyday moments a 45-minute session was never built to reach. It works under the supervision of our clinicians, and when someone needs more than it should handle, it gets them to a human. That's the point: Sona takes on what doesn't need a clinician, so clinical capacity reaches the people who do, without inflating healthcare costs. This report gets at the real issue: We’re not just facing a workforce shortage—we’re facing an infrastructure challenge. For substance use disorder care in particular, the path forward isn’t simply training more clinicians. It’s building care delivery systems that can provide high-quality, evidence-based care in a way that is also cost-effective and scalable. That requires rethinking how care is delivered: 1. Expanding who is part of the care team 2. Designing models that leverage peers and community-based providers 3. Embedding care where patients already are—especially in primary care As demand continues to grow and supply remains constrained, our community has to think more creatively—leveraging technology, new workforce models, and better-aligned incentives to close the gap. The workforce we need already exists. The opportunity now is to build the systems that allow it to operate at scale. Appreciate Meadows for helping move this conversation forward.
