As CMS launches its new Office of Rural Health Transformation, I wanted to spend some time digging into the supporting materials. There are some detailed summaries of newly announced programs. I’ll put links in the comments below. A few themes stood out to me: 1. A small group of states are leaning hard into systems change. Several states explicitly prioritize statewide care coordination, hospital stabilization, and digital infrastructure as core uses of funds, and not one-off programs. Examples include Alaska, Arizona, Arkansas, California, and Florida. 2. Others are growing their own rural health workforce. Delaware is a good example, launching its first medical school with a dedicated Primary Care-Rural Health track to build long term capacity. 3. Many states are tying rural strategy directly to value-based care. Even when it’s not labeled as such, proposals frequently reference total cost of care, Medicaid delivery reform, or alignment with Medicare and ACO models. This signals that rural transformation and VBC are becoming increasingly intertwined. Big picture: this new CMS office feels less like a temporary funding vehicle and more like a long-term on-ramp for rural participation in value-based care. Understanding how your state is approaching this will matter more than the headline dollar amounts.
