Having a good idea in healthcare is not the hard part. The hard part is building the business around it. OFFOR Health started because kids on Medicaid were waiting over a year for dental procedures. The clinical solution was simple: bring anesthesia teams into dental practices instead of making families travel to hospitals and surgery centers. None of that reaches a single patient without the execution. New reimbursement codes. Malpractice frameworks for office-based anesthesia. Insurance contracts across multiple states. Recruiting care teams for a model that didn't exist yet. That's the work most people never see. Why families are no longer missing work for emergency visits that didn't need to happen. Thousands of kids are now getting care closer to home. Communities are healthier because a few physicians built the company around the medicine. The ASA Monitor featured my co-founder Navin Goyal MD and the OFFOR Health story this month. This is our work beyond the check. Everything between a good idea and kids getting care. "Embracing the Unknown: Entrepreneurship in Ambulatory Anesthesia" linked in the comments.

One kid went to the ER 30 times just to get teeth extracted. Not because the solution didn't exist. Because the system couldn't deliver it. Meanwhile that kid is in pain. They can't eat. They're malnourished. They miss school. Their parent misses work. And the ER becomes the default plan, over and over and over again. The cost isn't just medical. It's human. It's economic. It's community-wide. Buffy Alegria shares this story in her conversation with Stacy Havener and it's one of the best real-world examples of what healthcare VC actually looks like on the ground. Her co-founder Navin Goyal MD, a former anesthesiologist, built an alternative care model for kids on Medicaid that takes treatment from "wait a year and drive hours" to "get treated now." That's the whole idea behind LOUD Ventures. Healthcare doesn't need more detached capital. It needs investors who understand the front lines and know how to navigate the messy parts — reimbursement, legacy systems, the stuff that stands between a great idea and a patient who actually gets helped. Because the question was never "Is this a great idea?" It's "Can this idea survive the real world?" A brand new episode feat. Buffy Alegria of Loud Ventures is out now on Billion Dollar Backstory. Find it wherever you get your podcasts. ......... 🎙 Thank you to Ultimus Fund Solutions and GemCap for supporting Season 3 of Billion Dollar Backstory!

“This past year, we anesthetized 15,000 children in five states without using a single operating room,” said Navin Goyal MD, the cofounder of SmileMD, a company expanding access to anesthesia services for children in underserved communities. The insurance company reached out to us for our business…. a dream for an entrepreneur but a harsh reality of a healthcare system that can’t provide care for those in need. One of the largest pediatric hospitals in the country has a backlog of 1500 kids. We started a business 11 years ago and are directly addressing this - we are taking care of about 90-100 kids a day in 5 states. We didn’t change the system overnight, but we did create an alternative model of care that enables procedures to be performed in offices with insurance companies, nurses, physicians, and paramedics, and with patients… all aligned. That experience pulled me deeper into entrepreneurship, healthcare innovation, and challenging the limits of “how things have always been done.” We will take care of over 15,000 kids this year, and I can’t wait to see how we do next year. Proud of the team at OFFOR Health!

I’ve rarely met anyone who can take a venture from 0 to 1, 1 to 10, and 10 to 100. What I have seen is this: when you understand your own strengths and the strengths of those around you, the gaps become obvious. And once you see the gaps, you can fill them. This is why I keep thinking about the value of a flexible executive who can adapt across operations, execution, revenue, and finance. I’ve brought two executives into two of my own companies and plugged others into portfolio teams. The impact has been real. If you miss the gap, you miss the growth. LOUD Collective LOUD Ventures

Today, I’m proud to announce that OFFOR Health is entering its 5th state- Pennsylvania. Thank you to UPMC for its partnership and action to help children who have long wait times for their procedures. We will get started in November! We will have taken care of 15,000 patients in 2025 and can’t wait to continue to serve more patients. For reference, we see more patients in a day than a mid-sized children’s hospital does in a year… and we use existing dental office space to get these kids taken care of. This company is now 11 years old and to share that it started with one patient scheduled in one month to having over 100 in a day. I’m most proud that despite leadership changes, financial struggles, and other challenges, we kept our care teams in place to continue serving the children who had no other options. *Don’t stop believing. *Don’t listen to people who can’t see your vision *Don’t think that progress is neat and clean *Don’t ask an expert *Don’t give up, because others will give you many reasons to stop. Below is a pic of me talking about the company in China back in 2018. This access to care issue is global.
