Neighborhood Health Center was established out of need for access to quality health care for low-income, underinsured, and uninsured Oregon residents. Our providers are patient-focused, community-minded, and eager to support the health needs of the entire family. They listen to patients’ needs and help them get the best care possible. Whether patients need primary care, family planning, pediatric, behavioral health, or dental care, we’re here for them. Our mission is building healthy communities – one neighbor at a time – through patient-centered health care, regardless of income or current state of wellness.

  • At NHC, family medicine looks a little different. You're not working alone. You're part of a team that includes behavioral health consultants, clinical pharmacists, nurses, and community health navigators — all working together to support the whole patient, not just the chief complaint. You're also not practicing for profit. About 75% of our patients are on Oregon Health Plan. They need consistent, trusted primary care from someone who will be there for them long term. That's the kind of medicine we're built around. We're also actively growing our prenatal care program, and we need physicians who are excited about supporting patients through pregnancy. This is outpatient prenatal care only, with no delivery responsibilities. We're hiring Family Physicians who want exactly that, including physicians comfortable providing prenatal care as part of their practice.   • Today is Chronic Disease Awareness Day. And for community health centers like NHC, it's a day that hits close to home. Oregon Health Plan members carry a disproportionate burden of chronic disease. The data from Oregon's Behavioral Risk Factor Surveillance System makes that clear: 🔹 OHP adults have asthma at nearly twice the rate of non-OHP adults (22.3% vs. 12.5%) 🔹 OHP adults are diagnosed with diabetes at a significantly higher rate (13.9% vs. 10.1%) 🔹 In Clackamas County, nearly 7 in 10 OHP adults are living with at least one chronic condition These gaps don't happen by accident. Chronic disease and economic hardship are deeply connected. Limited access to healthy food, safe places to exercise, stable housing, and consistent healthcare all shape health outcomes in ways that are clear in the data. At NHC, approximately 75% of our patients are on the Oregon Health Plan. That means chronic disease management is at the center of almost every care interaction we have. We manage diabetes, asthma, heart disease, depression, and more (alongside primary care, dental, and behavioral health). Not because it's easy, but because our patients deserve care that sees the whole picture. Community health centers exist precisely for moments like this: to stand in the gap between chronic illness and the systemic barriers that make it harder to get and stay healthy.   • When your own staff digs into their pockets to help patients, that says everything about your culture. NHC's #Proud2BNHC Employee Giving Fund is 100% funded and directed by our own employees. Every dollar goes directly toward non-covered medical and dental necessities for patients who could not otherwise afford them.   • What does it take to keep a Community Health Center running? Lately, we've had the chance to talk about it directly with policymakers. Over the past two weeks, our Canby Clinic hosted three visits — Senator Merkley's office, Congresswoman Janelle Bynum, and Representative Matt Bunch — for tours and conversations about the issues shaping healthcare access in our community. The topics on the table: 🔹 How proposed federal policy (HR1) could affect healthcare funding and access 🔹 Medicaid work requirements and eligibility redetermination 🔹 Why student loan burden is driving workforce shortages in healthcare 🔹 The growing need for behavioral health support, including in schools, and its impact on law enforcement 🔹 Rising uninsured rates and the strain on community health systems 🔹 340B pharmacy program challenges affecting patients' access to affordable medication These are not abstract policy debates for us; they are daily realities for our staff and patients. Conversations like these help policymakers understand what's actually at stake when these decisions are made. We are especially grateful to Representative Bunch, who offered to help connect NHC leaders with additional legislators to expand awareness of community health centers and the patients they serve (a generous next step we're looking forward to).   • Four years of building, training, and strengthening our team from the ground up. 💛 Please join us in celebrating Sara Campos, NHC's MA Trainer, on her 4-year work anniversary! As a Trainer, Sara plays a vital role in onboarding and developing our Medical Assistant team, making sure every new hire has the tools, support, and confidence to succeed in their role and grow their career at NHC.
