A new study in JAMA evaluated levels of #blood-based p-tau217, a marker of Alzheimer disease #brain changes, to estimate the risk of developing cognitive impairment over 2, 5, and 10 years among older adults without cognitive symptoms. The findings indicate that higher baseline plasma p-tau217 levels are associated with increased risk of #cognitive decline at 5 years (12% in the low group vs 38% in the very high group) and a faster rate of cognitive deterioration. Medicare-funded residency positions created through the Consolidated Appropriations Acts of 2021 and 2023 increased slots for health professional shortage areas and psychiatry, but fell short of legislative targets for rural placement and primary care prioritization. The proportion of positions in rural counties declined from 6% in round 1 of the 2021 Act to 3% in round 4, and only 1% in the 2023 Act, never reaching the targeted 10%. Although psychiatry slots rose and overall postgraduate year 1 positions increased in interventional radiology and psychiatry, primary care growth was modest.
