  • For all the precision isotretinoin demands, one of the hardest parts is knowing when a course is actually done. But as John Barbieri, MD, MBA, reminds us in this episode of The Spot Check, the real endpoint is both more human and more clinical than that. “The goal is to try to achieve a long-term remission of acne and get long-term clearance,” he says. The challenge is getting patients to a high enough cumulative dose to make that likely, without pushing past what they need or what they can tolerate. In part one of this Acne Awareness Month series, Jamie Restivo, PA-C, and Dr. Barbieri get into the details that change how isotretinoin is prescribed and counseled in practice: treatment endpoints, recurrence risk, once-daily dosing, absorption, micronized formulations, omega-3s, antihistamines, and the expectation setting that can make the course feel less mysterious for patients.
