#Fridayfindings #Caseoftheweek Bone Marrow Aspiration, Biopsy & Immunohistochemistry As hematopathologists, we occasionally encounter cases that remind us of the biological complexity of the bone marrow microenvironment. I would like to share a recent case highlighting the coexistence of Myeloproliferative Neoplasm (MPN) and Monoclonal Gammopathy of Undetermined Significance (MGUS) detected on bone marrow evaluation. 🧪 Clinical Context An elderly patient presented with persistent thrombocytosis and mild anemia. Serum protein electrophoresis revealed a low-level monoclonal spike, prompting a comprehensive bone marrow workup. 🔬 Bone Marrow Aspiration Findings · Hypercellular marrow for age · Marked megakaryocytic proliferation with clustering · Megakaryocytes showing atypical forms consistent with a myeloproliferative neoplasm · Plasma cells mildly increased (~5–7%), morphologically mature 🧬 Bone Marrow Biopsy Findings · Hypercellular marrow with preserved trilineage hematopoiesis · Increased and atypical megakaryocytes forming loose clusters · No significant fibrosis (reticulin stain: MF-0/1) · Interstitial increase in plasma cells without sheets or nodules 🧫 Immunohistochemistry · CD61/CD42b: Highlighted increased megakaryocytes · CD138: Plasma cells ~5–7% · Kappa/Lambda: Light chain restriction (lambda predominant) · CD34: No increase in blasts 🧾 Integrated Interpretation The overall findings were consistent with: · Myeloproliferative Neoplasm (likely Essential Thrombocythemia) · Coexistent MGUS This case underscores the importance of systematic marrow evaluation and immunohistochemistry in identifying dual hematologic pathologies, which may have distinct prognostic and therapeutic implications. 📌 Take-home Message The coexistence of MPN and MGUS, though uncommon, is a recognized entity and emphasizes the need for careful morphologic assessment and clinicopathologic correlation. Always fascinating how the bone marrow can tell more than one story at a time. 📌 Interesting Cytogenetic Profile in Chronic Myeloid Leukemia (CML) Chronic myeloid leukemia (CML) is characterized by the chromosomal translocation t(9;22)(q34.1;q11.2) resulting in the formation of the Philadelphia (Ph) chromosome. Significant improvement in prognosis and survival rate has been seen after the introduction of tyrosine kinase inhibitors (TKI) in the treatment protocol. But over the years of treatment, additional cytogenetic abnormalities (ACA) are known to crop up in Ph+ cells due to cytogenetic evolution. The most common additional abnormalities include trisomy 8 and trisomy 19 and extra copy of Ph chromosome and i(17) (q10). The evolution of these abnormalities in Ph+ cells predicts a poorer response to TKIs and a higher risk of progression. 📃 Case highlights: · Presence of t(9;22) translocation (Philadelphia chromosome) · Extra Philadelphia chromosome (Derivative 22 due ot 9-9;22) · Isochromosome 17q [i(17q)] 🎯 Why this matters: · An extra Ph chromosome indicates clonal evolution and is commonly associated with accelerated or blast phase CML. · Isochromosome 17q results in loss of 17p (TP53 locus) and gain of 17q, a change linked to genomic instability, disease progression, and poorer prognosis. · These abnormalities often suggest resistance to therapy and the need for closer monitoring or treatment modification. 🧠 Take-home message: Comprehensive cytogenetic and molecular analysis is essential in CML—not only at diagnosis but also during follow-up—to detect clonal evolution early and guide risk-adapted management. 📰 Educational reminder: CML is not static; cytogenetic evolution can significantly impact outcomes. It provides: · Direct evidence of a plasma-cell clone (often small and easily missed on routine tests). · Tissue confirmation of amyloid, establishing the diagnosis with certainty. · Critical information for staging and treatment planning, linking the monoclonal process to systemic involvement. 📌Essential point: A bone marrow biopsy can be the decisive test that converts ambiguous lab findings into a definitive diagnosis of AL amyloidosis—especially when the monoclonal protein burden is low or invisible on electrophoresis.
