About us

Community Healthcare Network is a not-for-profit organization providing more than 65,000 New Yorkers with primary and behavioral healthcare, dental, nutrition, wellness, and needed support services. Our network is made up of 13 federally qualified health centers throughout Brooklyn, the Bronx, Queens, and Manhattan, along with 2 mobile vans that bring health services to underserved people in need throughout New York City. We provide judgment-free, high-quality healthcare, without regard to race, religion, orientation, gender identity, immigration status or ability to pay. We turn no one away. Our health centers serve neighborhoods that are incredibly diverse. More than 96% of CHN’s patients are people of color, with 46% identifying as African-American/Caribbean, 49% as Hispanic, and 1% as Asian-American. A large number of our patients are immigrants and the working poor. Approximately 29% of CHN’s patients are best served in a language other than English.

  • Pride Month is a time to celebrate the LGBTQ+ community and reflect on the importance of creating spaces where everyone feels supported and respected. It also honors the legacy of the Stonewall Uprising of 1969, a pivotal moment in the fight for LGBTQ+ rights that helped spark the modern Pride movement. This year’s NYC Pride theme, “For All of Us,” reminds us that community, visibility, and care matter. 🏳️🌈 We’re proud to serve LGBTQ+ members of our community with care and services that meet their needs, including free HIV testing through our mobile units, PrEP and PEP, and family-building health education. We also offer dedicated programs like our Transgender Family Program, which provides weekly workshops and creates safe spaces in low-income communities for HIV prevention, care, and individual wellness.   • This May, we're celebrating the rich cultures and stories that make up the Asian American, Native Hawaiian, and Pacific Islander (AANHPI) community, including right here at CHN! 🩵 🌸 We asked some of our staff to share their reflections on AANHPI Heritage Month, and here's what they had to say. Stay tuned for Part 2!

  • We are proud to celebrate two meaningful milestones in Jamaica that highlight our continued commitment to community-centered care. On April 20th, 2026, CHN Sutphin Boulevard officially reopened its doors after nearly four years. Pictured here are CHN team members who were present on opening day, marking a long-awaited and meaningful return to the community. Patient reception has been overwhelmingly positive, with many commenting on the healing and beautifully redesigned atmosphere, which reflects CHN’s dedication to providing welcoming, high-quality spaces for care. Just weeks later, on Saturday, May 2nd, CHN partnered in the Annual Prostate Cancer Health Fair hosted by the First Presbyterian Church of Jamaica. CHN offered a range of free preventive health services to community members, including: • Eye screenings, courtesy of Dr. Charis-Ann Ricketts and Sermona Howlader • Blood pressure screenings, provided by Director of Nursing Wendy Garcia • Nutrition counseling and education, led by Sara Pellegrini, Nutrition Program Specialist • HIV testing and counseling, provided by David Guayambuco Parra and Denise Gutierrez Robles Together, these moments reflect the strength of CHN’s presence in Jamaica and our ongoing mission to support the health, wellness, and trust of the communities we serve.

    As VP of Nursing, I had the distinct privilege of contributing to my first clinic launch as CHN Community Healthcare Network opened the Sutphin Health Center, one of many health centers across New York City. This milestone represents far more than the opening of a new site, it reflects months of strategic planning, cross-functional collaboration, and a shared commitment to expanding access to high-quality, comprehensive care. Appreciation is extended to the collective efforts of Medical Affiars , Operations, Laboratory, Informatics, Facilities, IT, and Logistics teams, whose expertise and seamless coordination made this achievement possible. I am honored to work alongside such dedicated, mission-driven colleagues. A special shout-out to our Director of Nursing, site nursing leadership, and nurses for the outstanding planning and seamless transition in preparation for opening. Together, we continue to strengthen CHN’s ability to serve the community through integrated services, including pediatrics, adult medicine, women’s health and obstetrics, HIV care,behavioral health and many more.
