At some point in their lives, the majority of dialysis patients will need work on their fistulas. The disease in the veins is always the same; the webbing and scarring cause the stenosis to recur months later. The Serranator changed my approach. The stainless steel serrated strips are designed to create micro-fissuring along the vessel wall, like the release lines of a sketchbook page, allowing the vessel to open and remodel, which increases patency. This patient had complete central occlusion. Tibial arterial disease is one of the toughest areas in peripheral intervention with recoil, restenosis and a high rate of reintervention. Dr. Sagar Gandhi followed four different patients with 90-day outcomes demonstrating the durability of Serration Remodeling Therapy (SRT). 68M with a chronic second-toe ulcer and multiple prior anterior tibial interventions. Recurrent disease, including two recent POBA failures within weeks. Heavily calcified vessels with ABI of 1.7 and toe pressure of 32 mmHg. Following Serration Remodeling Therapy #SRT: ABI trended to 1.29 at 60 days, with a corresponding increase in toe pressure and complete wound healing.
