  • In an interview with Targeted Oncology, Meredith McKean, MD, MPH, of Sarah Cannon Research Institute reviews the Phase 2 imneskibart data presented at #ASCO26 in patients with doublet checkpoint inhibitor-refractory metastatic melanoma — a population with limited options once disease has progressed on potent frontline immunotherapy. Dr. McKean discusses a long-standing challenge in IL-2 therapy: harnessing its anti-tumor activity without the severe toxicities that have historically required inpatient management. In this study, imneskibart with low-dose, subcutaneous aldesleukin, with and without a checkpoint inhibitor (nivolumab), was administered entirely in the outpatient setting, with no treatment discontinuations due to drug-related adverse events.   • Beyond melanoma, imneskibart is advancing in non-small cell lung cancer — a setting where patients whose disease has progressed on checkpoint inhibitor therapy continue to face limited options. Two ongoing Phase 2 cohorts are evaluating imneskibart and low-dose, subcutaneous aldesleukin — with and without avelumab (anti-PD-L1 with an active Fc domain and ADCC effector function) — in patients with advanced PD-L1+ NSCLC that progressed on prior CPI therapy.   • The tumor microenvironment (TME) is only part of the imneskibart story. Beyond the TME, selective IL-2 engagement is designed to preserve immune activation while limiting the toxicities that have historically constrained this pathway, including vascular leak syndrome and pulmonary edema. Imneskibart prevents IL-2 from binding to receptors on the lining of blood vessels, blocking a key mechanism behind these serious side effects. Clinical data demonstrate that imneskibart can activate effector T cells and NK cells, reduce Tregs and support a more controlled immune response — contributing to a distinct profile within the IL-2 class.
